This Figure from 1969, long before the statin era, illustrates the natural history of untreated heterozygous FH. For males with untreated heterozygous FH, 51% will have a cardiovascular event by age 50 years; by the time a male with untreated heterozygous FH is 60, 85% will have a coronary event. Women in families with FH typically reach the same probability of having a CHD event about a decade later than men; as shown in the Figure, about 12% of women with untreated heterozygous FH will have had a cardiac event by age 50 years, and 58% will have an event by age 60. In summary this highlights that untreated heterozygous FH puts people at risk for very early CHD and CHD events at exactly the age when many people are at the peak of their working careers and raising their families.
