The treatment goal in all atherosclerosis, but especially in FH and in severe FH patients, is to reduce the number of ApoB-containing lipoproteins in the circulation, and the earlier in life that this can be achieved, the better.  This means that the goal should be to move these severe FH patients closer to the normal disease progression curve.  As seen in the Figure, the solid blue line is a homozygous FH patient in whom CHD occurs in the teens because of the extreme exposure to elevated cholesterol from birth.  In heterozygous FH patients, CHD occurs in their 40s.  The treatment goal therefore is to reduce ApoB-containing lipoproteins, specifically LDL, so that these patients can move to the right on this curve and have a normal lifespan.
