FIG. 21: Bilevel PAP with a spontaneous/timed backup rate (BiPAP ST) has been suggested as an effective alternative therapy for patients with Op-CSA.[20,21] There are small case-reports with complete resolution of CSA with BiPAP. In a head-to-head comparison of CPAP and BiPAP ST, Guilleminault and colleagues[21] showed that BiPAP ST was markedly superior to CPAP in the treatment of Op-CSA. Unfortunately none of these studies was able to isolate specific characteristics of patients that would predict successful CPAP versus BiPAP titration. Nonetheless, these findings suggest that BiPAP ST should be considered as a next step for patients with CSA who fail CPAP.
