FIG 13: Putting together the findings in this patient, he presented with bowel and cerebral infarctions, with a comorbidity of cardiac and cerebrovascular disease, both of which influenced his respiration. Although he presented with a waxing and waning, crescendo-decrescendo periodic breathing pattern (which is called CSR in heart failure patients), he also presented with paradoxical breathing, which means obstructive sleep apnea (OSA). What is the optimal treatment for this patient? Should we apply CPAP? Or should we apply ASV?
