After one month of medical stabilization but just after implantation of the CRT-D device, he had very severe central sleep apnea; AHI 63 with 90% central events with a varied sleep pattern of periodic breathing and long cycling, which indicated poor cardiac function.  At this time we decided not to treat this patient and instead to wait until he had received the maximal benefit of his cardiac optimization and his CRT-D.
