FIG. 28: The Rice study[21] was followed by the study by Fan et al,[22] which ran almost in parallel but was published 2 years later. This was also a VA cooperative study enrolling COPD patients at high risk for an exacerbation. The study recruited six VA sites, again randomized patients to a comprehensive care management program versus usual care. For the self-management treatment group, the intervention included four 90-min weekly individual sessions and one group session; in other words, this was a more intensive approach than that in the Rice study.
The Fan study included an action plan for identification and treatment of exacerbations, scheduled proactive telephone calls for case management, individualized written action plans for the exacerbations, contact information for a case manager, and a telephone number for the VA helpline similar to the Rice study. In summary, this was an even more intensive self-management education and intervention approach than applied in the Rice study.
