FIG. 39:  Sarcomatoid bladder carcinoma is even more rare than the other variants reviewed above, comprising only 0.3% of bladder cancer cases, although 5 sarcomatoid features are being found in up to 6% of bladder cancer patients.[7,22,23]  Sarcomatoid carcinoma also has a poor prognosis and marked aggressiveness.  Most cases are invasive.  Following upfront cystectomy, the 2-year survival rate is about 50%, but data are scarce, and no conclusions can be drawn about neoadjuvant chemotherapy.
