There is no substitute for good clinical judgment!

FIG. 20:  Toxicity and survival are both important, and there is no substitute for good clinical judgment.  If an ill patient cannot tolerate cisplatin-based chemotherapy, then proceeding with definitive treatment seems reasonable.  However, for a patient with muscle-invasive bladder cancer who can tolerate cisplatin-based neoadjuvant chemotherapy, this additional treatment can be life-saving.
