Small Cell Carcinoma

FIG. 37:  The fourth histologic variant that I will discuss is one that completely changes our management approach to T1 high grade, the presence of small-cell carcinoma.  These variants behave identically to undifferentiated small-cell carcinoma of the lung.  The variant is often associated with paraneoplastic syndromes, and has a high stage of presentation and frequent dissemination in patients.  It has a predilection for brain and bone.  Brain imaging is standard, as recurrence often happens in this sanctuary site.  The patients are not best treated with radical cystectomy but rather with neoadjuvant chemotherapy, despite the fact that they may be staged as T1 high grade.
