FIG. 33:  The NCCN guidelines[5] do not differentiate between the urachal and non-urachal  subtypes, but they also state that there is no proven role for neoadjuvant chemotherapy and that local control by cystectomy or, again, radiotherapy is important.  They stress the need for an en bloc removal in case of urachal cases, and for chemotherapy in advanced cases they suggest the FOLFOX regimen or gem-FLP (gemcitabine, fluorouracil, leucovorin, and cisplatin), then surgical consolidation in cases responsive to chemotherapy.
