FIG. 46:  A recent analysis[30] of a series of 40 cases of micropapillary bladder cancer in the UK and Ireland showed that the outcome is quite poor in LVI cases, so for those patients , an upfront early cystectomy is advised. For cases that are purely non-invasive and without LVI, potentially bladder preservation is feasible. However, this was a small series and currently upfront radical cystectomy is recommended in these patients.
